New Athlete Packet	This medical packet is required for all NEW student-athletes coming to CWU Athletics. It MUST be completed before participating in athletic practice and/or competition at the beginning of the current academic year.

Health Insurance Requirement	This document outlines CWU insurance requirements for student-athletes and what happens after a student-athlete is injured in-regards to medical bills and our secondary insurance policy.
ADHD Reporting Form	This form is required by NCAA to document a student-athlete's diagnosis of ADHD/ADD and must be filled out prior to the start of the academic year. This form is only required to be completed once during the student-athlete's time at CWU.
Prescription Medication Form	Required for all student-athletes that require the use of prescription medication for asthma, allergies, or diabetes that will be stored by, and provided to the athletic training room.
